HOW TO BECOME A PATIENT AND BEFORE YOUR FIRST VISIT:

Kindly watch the video to the left, this is very important and will give you an introduction to Dr. Winters and his practice, as well as what to expect on your first visit.

It is very important to send copies of insurance cards and any lab work associated with your health (X-Rays, MRIs, blood work, etc.) along with your New Patient Information forms. You may access the forms below the video.

It is of UTMOST IMPORTANCE to fax, email, mail or bring lab work, X-Rays, MRIs, Reports, and blood work to the office for Dr. Winters to review at least 24 hours BEFORE your appointment. This allows Dr. Winters time to review your file and be prepared for your exam.

New Patients

Please download, print, and complete the Adult New Patient History Form or the Pediatric History Form ( 16 years and younger ). Having the forms to us 24 hours before your first visit will help Dr. Winters be prepared to ask precise questions and target his exam. You can fax the form(s) to our office at 270-554-8795, scan and e-mail them to [email protected] or drop them by the office if you are local. If the forms will not be completed until your arrival you must arrive at least 45 minutes early. Our aim is to get unusually good results and that necessitates a thorough history.
